Charlotte Ann Madison Moulton, 84, of Ephraim passed away on May 22, 2022 in Provo, Utah.
She was born on Oct. 21, 1937 in Idaho Falls, Idaho to Truxton Dale and Lottie Jones Madison. She was married to Eugene Moulton.
Graveside services will be held on Wednesday, June 1, 2022 at noon in the Ephraim Park Cemetery. Viewings will be held on Tuesday, May 31 from 6-7 p.m. and Wednesday from 10-11:30 a.m. in the Ephraim 2nd Ward Chapel, 450 North 200 West.
