Ephraim Elementary kindergarten registration

Children who will turn 5 years of age on or before September 1, 2025 are eligible to attend kindergarten at Ephraim Elementary School this upcoming fall. Kindergarten registration will be held at Ephraim Elementary school on Wednesday, April 23 from 9 a.m. to 2 p.m. at the school, 570 South 300 East, Ephraim, UT.

If your student plans on attending kindergarten this upcoming fall, please call the school before registration day at 283-4171 to have your child placed on the 2025-2026 enrollment list.

On registration day, parents MUST bring their child, their current immunization record or exemption form and birth certificate. The process will take approximately 30-45 minutes to complete. Your child will have a kindergarten assessment (test), immunization review, vision screening and pre-registration paperwork.

Parents need a valid email address to complete the online registration. The Central Utah Public Health Department nurses will also be available at registration from 9 a.m.-2 p.m. for kindergarten students who need immunization.
